ESIC Form 32 is used for getting the benefit under the disablement scheme of ESIC.  This Form confirms the wage/contribution record of the employee.  On this Form, the employer has to write the name of the insured person, insurance number, local office to which the employee attached, date of entry, date of injury, name and address of the employer.

This Form specify the benefit period of employment in which the injury occurred.
