Employee State Insurance Corporation has prescribed Form 22 for releasing funeral expenses in case of death of the insured member of the ESIC.
When any insured person of the ESIC expires, then a nominee OR in the absence, the eldest surviving member of the family of the deceased insured person should submit a claim in the below-given format to receive expenses incurred towards the funeral of an insured person.
In the case of a minor, the guardian should sign the Claim Form on behalf of the minor and then submit it to the concerned authority of ESIC.
