ESIC Form 10
ESIC Form 10 is used for verification of the period of absent days/months of the employee/worker from Company/Factory and the reason may be whatever. In fact, this form is to be submitted to ESIC Office for the purpose of a claim under the Sickness Benefit, Temporary Disablement Benefit and Maternity Benefit Claim.
ESI Office uses the Ist part of this form to inquire about the absenteeism of the employee and write to the employer for confirmation. In turn, the insured person’s employer replies to the ESI office by confirming the sickness and number of days absent from the Company/Factory.
As an insured person under ESIC Scheme, the insured employee is not able to attend his job when he was suffering from any medical problem and taking treatment from ESI hospitals.
This Form 10 serves 3 purposes – Sickness Benefit, Temporary Disablement Benefit and Maternity Benefit Claim.
