ESIC Form 37 is issued by the employer in terms of the fact that the contribution towards the insured person has been deposited by the company and the employee is entitled to get all medical facilities and other benefits on the strength of this Form 37. After filling the above-downloaded Form 37, attest it from the employer/company and submit it to the ESIC regional/ ESIC local office and they will issue an ESIC Card to the Insured Person (I.P) for treatment of the sickness from the allotted Dispensary/Hospital.
This Form No.37 is of utmost necessity for those employees who are contributing to the ESIC regularly so that in case of any mishappening with the worker and dependant family may be benefitted immediately by ESIC.
Generally, it is also seen that Companies OR Organizations deduct the ESI contribution from the employee salary but not take a headache to provide them, ESIC Card, for getting medical facilities.
In such cases, the employee should visit the employer with Form 37 and after attestation from them, visit the ESI Dispensary/ESI Local office so that the IP and his family persons can be cured of their diseases and get medicines free of cost.
